To request an appointment please call our office at
813-295-5800 on Monday through Friday between 8am-5pm.

Please come half an hour prior to your scheduled time.

Established Patients

If you are an established patient and you need a referral to see the cardiologist, please make sure you have a current referral.

If you are unable to keep your appointment for office visits, ultrasounds, and scheduled office procedures, please notify the office 24 hours in advance. Failing which, there will be a $25.00 fee for non cancellations and no shows.

If your insurance has changed please let us know in advance.

The Patient Portal provides secure access to your electronic health records, and an ability to request appointments and communicate with us securely.
